Early Ages Family Day Care Online Enrolment Form
You may select the preference of Educator and preferred start date.
You may also use this process to be placed on a waitlist.

Minimum documents required are:
Immunisation record
Health record for allergies or asthma (if applicable).
Court order (if applicable).
What Happens Next
A friendly coordinator or your preferred Educator will be in contact with you soon.
Once enrolment is complete, an email will be sent to parent or guardian to complete enrolment process through their MyGov account.
You will also be required to confirm booking hours via email.
You will receive an Information Handbook and Harmony on enrolment.
You may be eligible for financial assistance such as the Child Care Subsidy to help cover the cost of fees. Apply for Child Care Subsidy prior to enrolment here if eligible. If you need more details Check out the ‘Resources’ page.
